A degree certificate is, in principle, issued only once and cannot be reissued. If you have lost your degree certificate or other graduation documents, you can request them from the UvA. If you wish to apply for a new degree certificate due to a change of gender, please contact the Education Desk or the Central Student Service Desk.

HR staff and employers can verify diplomas and certificates via the website of the Education Executive Agency (DUO). Alumni can, in many cases, download a diploma extract themselves through the register of the Education Executive Agency (DUO).

Many documents (including certified copies of degree certificates, confirmation of graduation letters, Confirmations of Enrolment and deregistration, and transcripts) can be requested via the Student Information System (SIS), the Education Desk and/or the Central Student Service Desk (CSSD). You can read how to request these documents on the student website.

Details of older degree certificates are not always available through the channels listed above. Registers up to approximately 1965 have been transferred to the City Archives. Please consult these archives for older graduation records.

Verifying graduation details

The UvA will only share your data with third parties if you give us written consent. A request must clearly state:

  • which personal data is involved
  • who is granting this consent
  • with whom this data may be shared

If we have any doubts about the authenticity of the consent or about the third party, we will take additional steps to verify your identity. Verification requests can be emailed to baliediv@uva.nl.

Lost degree certificate or transcript

  • If your degree certificate is not available through the above channels, you can request a certified (Dutch-language) extract from the examination register, which serves as official proof of graduation.
  • If certified degree certificates, transcripts, confirmations of graduation and/or enrolment are not available via the Education Desk or the CSSD, the Documentary Information Services (DIV) department may still be able to retrieve information or create documents. We cannot certify documents issued by other organisations (such as DUO). You can read below how to request these documents.

Application procedure

Send an email to baliediv@uva.nl including:

  1. A scan of your diploma
  2. The following details:
  • Initials and surname*
  • Date of birth (dd-mm-jjjj)*
  • Place and country of birth
  • Telephone number
  • Address: street, house number, postal code, city, and country
  • Student number
  • Degree programme and faculty*
  • Date/year of graduation
  • Any additional remarks

*Required fields

By submitting this information, you give permission to the UvA to process your data. Your data will be deleted within six weeks after your request has been completed. Consent forms for the sharing of personal data with third parties are destroyed five years after completion of the process.
